Step Up to Kindergarten

 

Dear Parent/Caregiver:


Thank you for participating in Step Up To Kindergarten. We are asking parents to fill out two screeners to help us in getting to know your child. The first screener ASQ-3 is a screener that will give us a quick feedback of your child’s overall development. The second screener is the ASQ:SE2 which is a screening tool that will provide a quick check of your child’s social-emotional development. This questionnaire asks questions about your child’s behaviors and social-emotional growth. The information you supply from both screeners will help reveal your child’s strengths, uncover any areas of concern.


In order to complete the ASQ screeners, please go to the School Website and go to Eisenhower’s website and click on the PARENT Tab.


Directions for completing the ASQ-3 Screener


The first section of the ASQ-3 includes 5 developmental areas.  Each area has 6 questions that go from easy to more difficult skills.  Your child may be able to do some but not all of the items- that is expected.  Read each question and then mark the answer as follows based on of your knowledge as their parent:


Yes—If your child is performing the skill


Sometimes—If your child is performing the skill but doesn’t yet do it consistently

Not yet—If your child does not perform the skill yet


The second section asks important questions about your child’s development and any concerns you may have about your child’s development. Answer questions by marking YES or NO, and if indicated, please explain your response.


Directions for completing the ASQ:SE-2 Screener


This screener asks questions about your child’s behaviors and social-emotional growth. The information you supply will help reveal your child’s strengths and uncover any areas of concern.  Please read each question and answer the questions based off your knowledge as the parent and mark the answers as follows:


Often or always- My child performs this behavior often or always.


Sometimes- My child sometimes performs this behavior (not consistently).


Rarely or never- My child rarely performs this behavior or has never performed the behavior.

If you have concerns, check the circle to the right of a question.

Some questions have blank spaces for you to provide examples of your child’s behavior. Please be sure to explain your response(s).
